UHC Rewards
In order to achieve compliance during the 2024 plan year and not be subject to the 6% wellness surcharge in the 2025 plan year, the following must be achieved:
- Complete a biometric screening by August 31, 2024.
- Complete the biometric screening results health coaching session by August 31, 2024.
- Complete the United Healthcare (UHC) health risk assessment by August 31, 2024.
For the 2025 plan year, those team members who obtain $350 or more in UHC rewards by August 31, 2024 of the 2024 plan year will receive the gold discount for the entire 2025 plan year.

Wellness Early Compliance Incentive
Team members who complete the biometric screening, health risk assessment, and results coaching session (30 min) by March 31, 2024, will receive a $100 gift card. This incentive is available to spouses on the plan.
Gold Discount
For the 2023 plan year, those team members who reach gold status in the month of July will receive the 3% discount on the total premium in the first pay period of August; those who obtain gold status in the month of August will receive their discount in the first pay period of September. For the 2024 plan year, those team members who reach gold status during the 2023 plan year (by September 30, 2023) will receive the 3% gold discount for the entire 2024 plan year.
