In conjunction with Corporate Health Partners (CHP) and Humana Go365, the City of Acworth is pleased to offer all eligible employees a comprehensive wellness program that strives to foster an environment that empowers our employees, families, and community to achieve their personal health and wellbeing goals by providing programs, tools, and resources that allow the mind, body, and spirit to thrive. A healthier you not only improves your quality of life, but also offers financial savings! WHAT IS IN IT FOR YOU? EMPLOYEES:

  • 2023 PREMIUM DISCOUNTS: Participants who are (or will be) covered by City of Acworth health insurance in 2022 must complete the below requirements by the specified deadlines to qualify for the lower premium in 2023.
  • 2022 BONUS PREMIUM DISCOUNT: Any employee achieving Gold Status in the Humana Go365 program on or before August 31st, 2022 will receive a 2% discount off their bi-weekly payroll deductions beginning the following month after achieving Gold Status. Discount will remain throughout the end of the calendar year.
  • ADDITIONAL $50 GIFT CARD: All participants who complete all of the following: BOTH Health Assessments (MyWellSite and Go365), Biometric Health Screening, and Results Coaching session by March 31, 2022, will receive a $50 Walmart gift card.
  • HUMANA GO365 BUCKS: The Go365 program rewards employees with Bucks you can use in the Go365 Mall for e-gift cards (Ex. Amazon, Target, etc.), the latest health activity trackers (Ex. Fitbit and Garmin) and more.

*Important: This requirement applies to all employees new to the medical plan and/or hired before April 29, 2022. Employees hired or new to the medical plan after this date are encouraged to strive for Silver Status before the end of the year in order to qualify for additional incentives the following plan year.

Here’s how to register with the Corporate Health Partners portal & app:

  1. Log on to
  2. Click on the Register button
  3. Employee Sign Up – Look up your account by providing:
    1. LAST NAME (DO NOT include suffixes, apostrophes, or spaces)
    3. User ID (Employees should use their Employee ID# and their spouses will use the same employee ID# preceded by “S”.)


  • If an employee did not participate or was non-compliance in the wellness program in 2021, the bi-weekly premium will be 5% higher.
  • If an employee is non-compliant with the 2022 wellness program, the 2023 bi-weekly premium will be higher than those employees who participated and completed all the requirements of the 2022 Wellness Program.
  • Although deadline dates for new employees hired after April 29, 2022 may vary for completing biometric screening, health assessments, and results coaching session, all requirements must be completed by October 31, 2022 to qualify for wellness discount in the 2023 calendar year.

City of Acworth is committed to helping you achieve your best health. Rewards for participating in a wellness program are available to all employees. If you think you might be unable to meet a standard for a reward under this wellness program, you might qualify for an opportunity to earn the same reward by different means.
